Once you get your referral and Appointment Control Number from United Concordia:
Find a network dentist and book your appointment directly with the dentist, or call a Dental Care Finder at 1-866-984-2337 to book the appointment for you. Directly below, is a list of Miltary Trusted Dentists on Oahu.
Once Your Appointment is Scheduled, Go To Your Appointment
Take your referral form with your Appointment Control Number. You can only get dental care that is listed in the referral. If the civilian dentist decides you need care that isn’t included on the initial referral, please tell him/her to contact your military dental clinic to get approval. If you follow this process, you’ll have no out-of-pocket expenses or paperwork to file.
